• Provide strategic leadership for resident care in the community/facility by working with each department to coordinate care and services that promote patient’s quality of life. Direct management of the nursing team and medication care managers, as applicable by State/Province law, to promote the health and wellness of the resident population.
• Licensed RN in good standing with State Licensing Authority
• Provide clinical care through the direct application of the nursing process; perform and document resident assessments, complete Individualized Service Plans, provide or delegate hands-on clinical care as indicated by the plan of care and evaluate patient outcomes.
•  Provide oversight of the community/facility medication management program to promote resident safety in the medication use process.
• Collaborate with physicians, pharmacists, and other clinical providers to coordinate care and services for the residents within the community/facility.
• Provide clinical training and education as needed to nurses, medication care managers, care managers and others who provide resident care.
• Perform skill evaluation to develop and promote clinical capabilities of team members.
• Track, trend, and report clinical quality data to identify risk. Actively participate as a member of the community/facility Quality Assurance and Performance Improvement committee. Lead clinical quality and process improvement initiatives within the community/facility to mitigate risk and improve resident care outcomes.
• Recruit hire and train clinical team members and is responsible for performance management, evaluation, and engagement.
• Completes direct report team member staffing and scheduling according to operational and budgetary guidelines.
• Support regulatory compliance for the community and clinical items under their purview.
• Clinical event management and response.
• CPR Certified

LPNs plans and manages patient care according to each individual’s needs. Examples of duties may include but not limit to:

• Recording health details of Residents; including vitals & temperature.
• Administering medications and injections to Residents as needed. • Treating and dressing wounds and bedsores as needed.
• May be required to supervise

• Must be able to communicate effectively with residents, families, staff, community and State officials and general public
• Must have compassion for and desire to work • Must demonstrate the ability to work responsibly as a team member as well as an individual
• Must have exceptional organizational skills and ability to re-prioritize daily tasks in order to accommodate fluctuating needs of the resident and Community
• Must have strong documentation skills
• Must meet all health requirements and pass background checks

• Build meaningful relationships with a specified number of patients and their families as you provide assistance with activities of daily living, attend to individual care needs and get to know their unique preferences and personalities
• Notify management of changes in condition and recommend adjustments in the level of care and service – Assist residents in life skills and other life enriching activities as indicated on their individual profile – Blends a variety of multi-sensory experiences into the resident’s day
• Participates in the development of the Individualized Service Plans (ISP) and monthly updates
• Responsible for their designated group of residents during the shift, knows where their residents are and physically checks on them throughout the shift.
• Partners with community team to ensure community is in compliance with OSHA requirements and promotion of Risk Management programs and policies; adherence to safety rules and regulations.
• Responds to the dining needs of the residents and guests while maintaining hospitality and service standards
• Invite, Encourage, Assist (IEA) residents in life skills and other life enriching activities as indicated on ISP and demographic profile
